The answer is (d) – it might be our environment or it could have been fixed by some other fault fix in the new version.
I have 10 years experience as a software developer and sometimes it's almost impossible to reproduce a fault. I think its important that testers log faults as clearly and concisely as possible. In my company we work very closely with our Test team so they often attempt to reproduce faults which we can't!
